4. Special Notes for Direct Primary Care & Self-Pay Patients

For patients enrolled in our Direct Primary Care program, no claims are submitted to insurance. Your records are still protected under HIPAA.

If you request that services not be billed to insurance (self-pay), we will honor your request in writing.
5. Changes to This Notice
We reserve the right to update or change this Notice at any time. Any changes will be posted at our office and on our website at www.crabtreehealth.com/hipaa.
6. Questions or Complaints
If you believe your privacy rights have been violated, you may file a complaint by contacting our office or with the U.S. Department of Health and Human Services (HHS). You will not be penalized for filing a complaint.
